Today we are talking about a disease that is spreading very fast in the world at the moment. What we all know as diabetes. Diabetes is becoming an epidemic with each passing day. In 2012, an estimated 28 million people in the US had diabetes mellitus, of which about 2 million have Type 1 diabetes and the remaining have Type 2 diabetes.

What is diabetes?

 There is an organ in our abdomen which we call pancreas. The functions of the pancreas is to make insulin and excrete it as required, this helps in food digestion and endocrine .its regulates insulin level in our body. 

So because of that we able to store fat or sugar in our body. When this pancreas stops making or releasing insulin, the amount of sugar in the body increases due to the lack of insulin.


There are many types of diabetes, but here we will talk about 3 main types.

Types of diabetes mellitus 

1.Type 1 diabetes 

2.Type 2 diabetes

Type 1 diabetes

Type 1 diabetes is immune - mediate in over 90% of cases (type 1a) and idiopathic in <5 5% (type lb).It occurs at any age but it is more common in children and young adults with a peak incidence before school age.

It is a catabolic disorder in which circulating insulin is virtually absent, plasma glucagon is elevated, and the pancreatic B cells fail to respond to all insulin stimuli. Insulin is therefore required to reverse the catabolic state, prevent ketosis, reduce the hyperglucagonemia and reduce blood glucose.


Type 2 diabetes

This type occurs mostly in adults. In this type, the pancreas suddenly reduces or stops making insulin. Genetically and environmental conditions also play an important role in increasing the risk of diabetes.

Obesity is the most important environmental factor which causes assistance of insulin in the body and causes diabetes mellitus 2. 30% of Chinese and Japanese population is suffering from diabetes mellitus 2. It is found 60 to 70% of North American.


Gestational diabetes

This type of diabetes is more common in women if their blood sugar goes up the normal level during pregnancy. It is very dangerous for both mother and the child. It can lead to abortion or preterm delivery or congenital defects in babies.

These women should have a regular follow up at their antenatal clinic monitored at the same time with an endocrinologist up till delivery and postnatal period.    

Tests  for diagnosing diabetes mellitus 
  • Fasting blood sugar (FBS)  The blood sugar level for normal fasting (no food for 8 hours) is between 70 and 99mg/dl,
  • Random blood sugar (RBS)  The normal level is less than 140mg /dl (7.8mmol/L)after 2 hours of eating.
  • Glycated haemoglobin test  (HbA1c test)   HbA1c is a blood test for type 2 and prediabetes. The test result of HbA1c is given as a percentage. The higher the percentage, the higher the level of the blood sugar. There is a standard HbA1c level below 5.7 percent. 
Prediabetes is in the 5.7 to 6.5 percent range. A risk factor for developing type 2 diabetes is having Prediabetes. Every year, people with prediabetes will require re-tests. Type 2 diabetes is above 6.5 percent. If someone has diabetes, you should have the A1C test at least twice a year.


Test for Gestational diabetes 
  •   Oral glucose tolerance test (OGTT)  Normally, gestational diabetes is tested between the 6th and 7th month of pregnancy, or a past history of miscarriage, using a test called the Oral Glucose Tolerance Test. 
A fasting blood sample is taken with a sample of urine (urine sample is important for check ketone bodies and sugar rollout). For adults, WHO suggested 75 g of glucose load and 1.75g / kg of body weight for infants, up to a limit of 75 g of glucose. In pregnant women, this procedure is completed in 3 to 4 hours.

Complications

Complications of diabetes: Diabetics need to be very careful because if diabetes is not taken seriously, it can lead to more dangerous diseases.  These include heart disease, osteoporosis, poor eyesight, kidney failure, etc. Many complications can be avoided if diabetes is well controlled.


Preventions of diabetes mellitus 

By lifestyle modification, which includes  

  • Daily exercise  (Walking 30 minutes in morning and evening)    
  •  Food is rich in protein and less in carbohydrates and fats.   
  • Avoid alcohol  
  • stop smoking  
  • Eating fresh fruits and vegetables 
  • Increase the amount of water intake   
  • Use natural sweeteners  
  • Decrease amount of tea or coffee  
  • Avoid stress 
  • Taking  medications regularly

In the world, over 85% of cases the chronic obstructive pulmonary disease (COPD) occur among current or former smokers.

Although cigarette smoking constitutes the most serious common medical problem, almost 45% of smokers attempt to quit every year, but only 5% are successful. People who have a face to quit smoking should exercise physically as well as the lungs.  

Exercise is very easy. Get up early in the morning, take long deep breaths in the open air, inhale oxygen from the nose and exhale through the mouth. This process lasts for at least five minutes. It will also start to heal the damaged lungs.

Today we are going to describe a disease that almost everyone has to deal with that. It is allergic rhinitis. What is allergic rhinitis? and how we stay away from allergic rhinitis?
  
Allergic rhinitis 
Rhinitis comes from  Greek word rhin- means nose, itis - means inflammation, therefore it is called nasal inflammation. Inflammation of the nose is a common disease. Our body has an immune system which kills bacteria and germs when this immune system begins to eliminate things that are not harmful to us .develop allergic rhinitis.

In seasonal allergic rhinitis, pollen is the most common allergen. When the seasons change allergy symptoms occur. Allergic rhinitis is also called hay fever because is caused by allergens hay for example dust, pollen, pet dander, etc.
   
It may seem like a minor disease. Rhinitis has not a complete cure, but we can control it to some extent with the help of medicines.   

At the result of this overreaction develop inflammation in the nose allergic rhinitis occurs, and the chemical that is produced during this allergic reaction is called histamine.

The patient is unable to concentrate at work due to rhinitis allergy. Allergic rhinitis adversely affects school and work performance. 

Allergic rhinitis can also affect the throat and ears. Patient with rhinitis can't sleep properly because their nose closes repeatedly, which makes it unable to breathe. Rhinitis should never be ignored because if not treated in time, it can turn into asthma. 

Treatment

First, the best treatment is to avoid triggering allergens. There are three types of medications for allergic rhinitis.
                                                                        Antihistamine   

Antihistamine offers temporary, but immediate control of many of the most troubling symptoms of allergic rhinitis. It can be taken orally or nasal spray.  

Montelukast   

This medicine has also proved to be very useful in rhinitis.

Intranasal corticosteroids

Intranasal corticosteroids sprays have revolutionized the treatment of allergic rhinitis.

Nasal sprays

Nasal saline sprays are very useful in the treatment of allergic rhinitis to mechanically flush the allergens from the nasal cavity.   

Precautions

Avoiding exposure to airborne allergens is the most effective means of alleviating symptoms of allergic rhinitis. Maintaining an allergen-free environment by covering mattresses and pillows with plastic covers.
  
When someone finds out that they have rhinitis. He should take immediate precautions. For example, if he is going to a dusty place, he should cover his face and nose.

Take special care for the cleaning. Patients with rhinitis should not stop taking medicines immediately when they recover, slowly reduce the dose first. Remember that medicines for allergic rhinitis don't have any specific side effects. 

Inflammation of the nose is not a deadly disease but it should not be ignored as it is an inflammation of the nose which can cause asthma. Therefore, a doctor should be consulted in time.

This is the year that taught the world a lot, this is the year that snatched a lot from the world - there will be no person or family who has not suffered in any way. This year, there is no home or individual who has not suffered in any way, COVID-19's patient or victim's family may have been or are going through an ordeal.

Most of us are at home with our families, police, health workers and cleaners are working tirelessly to protect us. They chose selflessness over self-preservation. They all are the real heroes and front liners against coronavirus pandemic. Today we pay tribute to a class who not only faced this dangerous epidemic day and night but also helped others.

There is a significant risk to people who are in the front-lines. These doctors and paramedics of ours are making unforgettable sacrifices to save the lives of human beings. They protect us and give us the courage to fight against COVID -19.

For the patient of COVID-19, as much as his family is worried, these front liners are just as upset. Regardless of their lives, these health workers are serving the people. We must not forget that doctors and paramedics are also human beings, their family members are also worried about them.

The doctors and paramedical staff are busy treating the patients. We admire their passion towards their duty. 

They are also running awareness campaigns on how to control the epidemic. The names of these doctors and nurses from all over the world will be written in golden letters in history. The epidemic is still in its place. We must help the doctors and paramedical staff in every way. 

For all the front liners against COVID -19, I would like to say thank you to all of you and your families. In this kind of crisis, we are facing today, front-liners give all their best; they choose to serve people instead of staying at home with their families.

We need to help each other in every possible way; we believe that soon we will be free from this deadly pandemic. I salute to all the people that follow the rules for their safety and for their trust to the front-liners that facing this deadly COVID 19.

People should follow SOPs as much as possible and stay in your homes and stay safe. Hope this epidemic will end soon, ۰we salute all the doctors and paramedics all over the world.
